What you need to have:

[ ] Diagnosis Report(s)
- For under-6 years old, you should have three reports from the psychologist, pediatrician and speech-language pathologist.
- For those over 6 years old, you should have one report from a psychologist or pediatrician.
[ ] BCAAN Diagnose Form or Non-BCCAN (Private) Diagnose Form completed by the assessor.
[ ] Make photocopies and/or scan both
[ ] Child’s birth certificate or other formal documentation with their birthdates.
[ ] Proof of residency (BC address) on the guardian’s ID
[ ] Application for Autism Funding (CF0906)
